Here you will find all the documents you need for furry friend to stay at A Dog’s Dream. With our interview process, we have a good deal of paperwork that needs to be filled out. Below you will find the forms needed. That way you can fill them out in the privacy and comfort of your home before you bring your dog in for their interview. Please bring each of the above forms with you to your Dreamer’s Interview. We also ask that you notify us of any changes to the information provided as soon as possible.
Vaccination records are required with your paperwork. Please have it faxed to 252-514-2507 or emailed to info@adogsdreamnc.com.
General information about you and your dog including emergency contact and veterinarian information. Please fill out completely and sign.
Policies for our Grooming Salon. Please sign and date. We must have a grooming contract on file before any work can be done.
If someone else will be picking up your dog we must have a signed release form on file.
Details regarding our care, responsibility, and payment policies. Please initial all highlights.
NOTE: Submitting these forms does not guarantee your dog a reservation. All dogs must pass a ‘Dreamer’s Evaluation’ and have health records on file. Please call us to confirm your appointment at (252) 514-2502.
